Required for Your First Visit
If this is your first visit to Cedar Pearl Dental, you’ll need to complete our new patient registration forms. These forms collect important information about your dental history, medical conditions, current medications, and insurance coverage. To make your visit as efficient as possible, we encourage you to complete our patient forms before your appointment. Arriving with completed paperwork allows us to spend more time focusing on your dental care and less time on administrative tasks.
Access your forms through our secure patient portal by clicking “Get Forms” below. Simply sign in, complete the forms online, and submit them electronically before your visit. If you prefer, you can also arrive 15 minutes early to complete forms in our office. Either way, our team is here to help if you have any questions about the information requested.
Please bring a valid photo ID and your insurance card to your first appointment. If you have dental X-rays from another office taken within the past year, we may be able to use those records to avoid unnecessary radiation exposure. Contact us in advance if you’d like to have records transferred from your previous dentist.

We accept most major insurance plans for our military families. Our insurance forms help us verify your coverage and process claims efficiently. We’ll review your benefits before treatment and provide cost estimates so you know what to expect.

For patients utilizing financing through CareCredit, additional financial forms may be required. Our team will guide you through the financing application process and help you understand your payment options. We also offer our Cedar Pearl Membership for patients without dental insurance.
